Here's something worth knowing on day one: for diabetic cats, food isn't just fuel — it's one of the most powerful tools you have. Cats are obligate carnivores, built to run on protein and fat rather than carbohydrate, and current veterinary consensus (including the ACVIM 2022 feline guidance) favours a low-carbohydrate, higher-protein diet for diabetic cats. Get this right early and you're giving your cat the best possible chance — including, sometimes, of remission.

Why does carbohydrate matter more for cats than dogs?

Because a cat's metabolism isn't designed to process large amounts of carbohydrate. Unlike dogs (which are omnivores and handle mixed macronutrients fairly flexibly), cats are obligate carnivores — their digestive system evolved for hunting and eating whole prey, which is protein and fat with minimal carbohydrate. A high-carb diet keeps blood glucose elevated and forces the pancreas to work harder, making diabetes worse. Reducing dietary carbohydrate directly lowers the glucose load the body has to manage — which is why it's so powerful for feline diabetes control.

What the current guidelines actually recommend

The most recent guidance is more specific than "feed low-carb". The 2025 iCatCare consensus guidelines on feline diabetes recommend transitioning to a diet with a carbohydrate content below 25% on a dry matter basis, below 15% of metabolisable energy, or under 5g per 100kcal — thresholds you can check against a food's label using our carbohydrate calculator. These guidelines also encourage a slow transition to a low-carbohydrate wet food where appropriate, rather than an abrupt switch.

Separate veterinary guidance has put the preferred profile slightly differently — high protein at 40% or more of metabolisable energy, with carbohydrate at or below 12% of metabolisable energy. The exact numbers vary between sources, which is worth knowing: the principle (meaningfully lower carbohydrate, higher protein) is well established, while the precise target is something your vet will set for your individual cat.

Diet, weight and the chance of remission

Weight matters alongside carbohydrate content. Research presented at the 2026 Western Veterinary Conference reported on a two-centre randomised trial of 72 overweight, insulin-treated cats, finding that a purpose-formulated low-digestible-carbohydrate diet combined with structured calorie restriction substantially increased short-term diabetic remission within a 12-week window. Notably, even where remission wasn't achieved, improved glycaemic control and reduced insulin requirements were still likely outcomes.

Veterinary guidance also indicates that achieving effective control of high blood glucose within six months of diagnosis increases the potential for remission — which is why the effort in those early months genuinely matters.

Any calorie restriction for a diabetic cat must be planned with your vet. Weight loss that is too rapid carries its own risks, and insulin needs change as weight changes.

Wet food or dry food?

Wet (canned) food is often naturally lower in carbohydrate than kibble, which typically needs starch to hold its shape and texture. Most commercial low-carb feline diets are canned for this reason. Wet food also supports hydration, which matters for kidney health in aging cats. That said, the perfect diet your cat refuses helps no one: the right food is a low-carb one your cat will actually eat, consistently. Here's the trap: most cat foods don't list carbohydrate directly. The label shows "guaranteed analysis" with protein, fat, ash, fibre, and moisture — but carbs are hidden. You calculate them:

Example: If a food shows 10% protein, 8% fat, 8% ash, 3% fibre, 75% moisture: Carbs = 100 − (10 + 8 + 8 + 3 + 75) = -4% (impossible, so try again — this label is unusual)

Better example: 40% protein, 18% fat, 8% ash, 3% fibre, 10% moisture: Carbs = 100 − (40 + 18 + 8 + 3 + 10) = 21% carbs

Transitioning food safely: a 1-2 week plan

Rapid food changes cause vomiting and diarrhoea — and that disrupts insulin absorption. Transition slowly:

Days 1-2: 75% old food + 25% new
Days 3-5: 50% old + 50% new
Days 6-10: 25% old + 75% new
Day 11+: 100% new food

If your cat vomits or has loose stools, stay at the current ratio for an extra 2-3 days. Some cats need a full 2-3 week transition.

Common feeding mistakes and how to avoid them

Free-feeding (leaving food out all day) is dangerous for a diabetic cat because you can't track intake and can't coordinate insulin timing. Meal-feed instead: set portions at set times, twice daily if possible.

Mixing old and new food incorrectly — don't assume ratios. Measure: if your cat eats 100g daily, day 1 is 75g old + 25g new, not "a bit of each."

Changing insulin timing mid-transition — Insulin timing is usually set to the old feeding time. Wait until the cat is fully transitioned before moving meal times, or work with your vet on a plan.

Skipping a meal because your cat refuses it — One skipped meal is tolerable. Multiple is a problem. Tell your vet; there are strategies (warming food, mixing in fish oil, appetite stimulants, trying different textures).

  • Try a different brand or texture: Some cats prefer pâté, others chunks. Some accept gravy-heavy foods better. Ask your vet for alternatives.

  • Mix with something irresistible: A tiny bit of fish oil, tuna juice, or a squeeze of a high-value topper.
  • Go slower: A 3-week transition instead of 1-2 weeks gives the palate time to adjust.
  • Talk to your vet about appetite stimulants: Short-term medication can help break the refusal cycle so the cat tries the new food.

Starving a cat to force diet compliance is dangerous — cats can develop hepatic lipidosis (liver disease) from not eating. Work with your vet on a plan that works for your cat.

When comparing cat foods, you'll often see two numbers for carbohydrate content: "as-fed basis" (the percentage in the can or bag) and "dry matter basis" (the percentage if you removed all the water). Understanding the difference is crucial.

Example: A canned food might list 10% carbs as-fed (because it's 80% water), but that same food would be 50% carbs on a dry-matter basis. Why does this matter? Because dry-matter basis is how nutritionists compare foods fairly — it shows you the actual nutrient density.

Real-world feeding obstacles and solutions

Your cat has other medical conditions: Some cats have kidney disease or urinary issues that complicate feeding choices. Work closely with your vet — a low-carb diet designed for diabetic cats might need adjustment. Never force a diet that conflicts with other health needs.

Cost concerns: Prescription low-carb foods are expensive. Ask your vet whether non-prescription options exist that are still low-carb. Some owners find quality store-bought pâtés with simple ingredients meet their cats' needs and are cheaper than prescription diets. The best diet is one you can afford to feed consistently.

Multi-cat households: If you have both diabetic and non-diabetic cats, feeding separately is ideal so the diabetic cat gets consistent meals. Use separate rooms or timed feeders so each cat gets the right food.

  • Your cat is losing too much weight: If on a low-carb diet your cat drops weight rapidly, tell your vet. You might be underfed or the diet might need adjustment. Losing weight is sometimes expected, but rapid loss warrants a check.
  • Glucose is improving rapidly: This is good, but it can also mean insulin needs to drop — discuss with your vet. Sometimes as glucose control improves, cats need less insulin even before remission.
  • Your cat's appetite is declining: If your cat eats less and less, this isn't normal diabetes. Could indicate a problem with the food or another health issue.
  • Cost is unsustainable: If you can't afford the prescribed diet, tell your vet immediately. They can work with you on alternatives or clarify whether a slightly higher-carb food is acceptable for your situation.

The role of weight management

While diet is central to feline diabetes management, weight matters too. Obese cats are more insulin-resistant; a healthy weight (determined by your vet) improves insulin sensitivity. Diet supports weight management, but:

  • Don't underfeed: A cat eating 150 calories a day when their ideal is 200 will lose weight but feel hungry and stressed. Work with your vet on target calories.

Could the right diet help my cat come off insulin?

In cats, combining a low-carbohydrate diet with good early glucose control and a healthy weight gives the best chance of diabetic remission, where the cat maintains normal blood sugar without insulin. Remission is never guaranteed and is not a reason to skip insulin or vet care — but it is real, and early consistency is what earns the chance. Many remission cats stop needing insulin within 4-12 weeks of starting a low-carb diet and achieving good control.

Signs of remission-readiness:

  • Glucose readings consistently in the 100-150 range without insulin
  • Pre-breakfast readings staying stable and moderate
  • Stable weight on a low-carb diet
  • Normal thirst and urination

Your vet will guide you through the insulin-weaning process if remission becomes possible. It's slow and careful — never stop insulin suddenly or on your own.
